Advanced Health & Wellness Services is a clinician-owned and operated organization dedicated to providing compassionate, person-centered support to adults age 65 and older and adults ages 18–64 living with disabilities. Our multidisciplinary team of nurses, social workers, clinicians, and care coordinators works alongside clients, families, healthcare providers, and community partners to ensure each individual receives coordinated, comprehensive support. We take a holistic, wraparound approach to care—helping with hospital-to-home transitions, care and treatment planning, risk and safety assessments, Medicaid recertification, durable medical equipment (DME), family coordination, and connections to essential healthcare and community resources. At the heart of our work is a genuine commitment to the people we serve. We believe every individual deserves to be heard, respected, supported, and cared for with dignity. Our goal is simple: to bring the pieces of care together so our clients can live as safely, independently, and fully as possible.
